Plavix - clopidogrel bisulfate

  Plavix (Clopidogrel) is used to prevent strokes and heart attacks.

Clopidogrel is an oral antiplatelet agent (thienopyridine class) to inhibit blood clots in coronary artery disease, peripheral vascular disease, and cerebrovascular disease. It is marketed by Bristol-Myers Squibb and Sanofi-Aventis under the trade name Plavix, by Sun Pharmaceuticals under the trade name Clopilet, by Ranbaxy Laboratories under the trade name Ceruvin. It works by irreversibly inhibiting a receptor called P2Y12. Adverse effects include hemorrhage.

Plavix is a pro-drug whose action may be related to adenosine diphosphate (ADP) receptor on platelet cell membranes. The IIb/IIIa complex functions as a receptor mainly for fibrinogen and vitronectin but also for fibronectin and von Willebrand factor. Activation of this receptor complex is the “final common pathway” for platelet aggregation, and is important in the cross-linking of platelets by fibrin.

Platelet inhibition can be demonstrated two hours after a single dose of oral clopidogrel, but the onset of action is slow, so that a loading-dose of 300-600 mg is usually administered.

Pharmacokinetics and metabolism

After repeated 75-mg oral doses of clopidogrel (base), plasma concentrations of the parent compound, which has no platelet inhibiting effect, are very low and are generally below the quantification limit (0.000258 mg/L) beyond 2 hours after dosing.

Clopidogrel is a pro-drug activated in the liver by cytochrome P450 enzymes, including CYP2C19. The active metabolite has an elimination half-life of about 8 hours and acts by forming a disulfide bridge with the platelet ADP receptor. Patients with a variant allele of CYP2C19 are 1.5 to 3.5 times more likely to die or have complications than patients with the high-functioning allele.678

Following an oral dose of 14C-labeled clopidogrel in humans, approximately 50% was excreted in the urine and approximately 46% in the feces in the 5 days after dosing.

Effect of Food: Administration of PLAVIX (clopidogrel bisulfate) with meals did not significantly modify the bioavailability of clopidogrel as assessed by the pharmacokinetics of the main circulating metabolite.

Absorption and Distribution: Clopidogrel is rapidly absorbed after oral administration of repeated doses of 75 mg clopidogrel (base), with peak plasma levels (appx. 3 mg/L) of the main circulating metabolite occurring approximately 1 hour after dosing. The pharmacokinetics of the main circulating metabolite are linear (plasma concentrations increased in proportion to dose) in the dose range of 50 to 150 mg of clopidogrel. Absorption is at least 50% based on urinary excretion of clopidogrel-related metabolites. Clopidogrel and the main circulating metabolite bind reversibly in vitro to human plasma proteins (98% and 94%, respectively). The binding is nonsaturable in vitro up to a concentration of 110 ?g/mL.

Metabolism and Elimination: In vitro and in vivo, clopidogrel undergoes rapid hydrolysis into its carboxylic acid derivative. In plasma and urine, the glucuronide of the carboxylic acid derivative is also observed.

Clopidogrel is an anti-platelet drug, that is, a drug that inhibits the ability of platelets to clump together as part of a blood clot. It is similar to ticlopidine (Ticlid) in chemical structure and in the way it works. Unlike ticlopidine, clopidogrel does not cause serious reductions of white cells in the blood and, therefore, routine blood testing to determine if the white blood cell count is low is not necessary during treatment. The risk of heart attacks and strokes (which usually are caused by blood clots) is increased in patients with a recent history of stroke or heart attack and patients with peripheral vascular disease. (Peripheral vascular disease is the same as atherosclerotic arterial disease or “hardening” of the arteries in which the arteries become narrowed. It frequently occurs in the legs and often causes claudication or pain in the legs upon walking). Clopidogrel is used to reduce the risk of heart attacks and strokes in these patients. Plavix was approved by the FDA in 1997.

If the incision is made under the arm, the surgeon may use a probe fitted with a miniature camera, along with minimally invasive (very small) instruments, to create a “pocket” for the breast implant.

Periareolar This incision is the most concealed, but is associated with a higher likelihood of inability to successfully breastfeed, as compared to the other incision sites.

Inframammary This incision is less concealed than periareolar and associated with less difficulty than the periareolar incision site when breast-feeding.

Axillary This incision is less concealed than periareolar and associated with less difficulty than the periareolar incision site when breast-feeding.

The breast implant surgery itself may be done either under local or general anesthesia in the hospital, a surgical center, or in the physicians office. The procedure is usually done as one-day outpatient surgery; however, for some women an overnight stay is recommended. The procedure takes two hours or more, depending on whether the implant goes above or beneath the muscle.

First, the surgeon creates a pocket-a space between the tissues-to accommodate the implant. After inserting the implant, he or she closes the pocket and sutures the tissues in place. Then either a surgical bra or bandage is placed over the incision. Post surgical swelling is to be expected, as is some pain and discomfort. Most women return to work and resume non-strenuous activities, such as driving, about a week to ten days after surgery.

Symptoms of acid reflux may be typical or atypical. But based on the diagnosis of acid reflux patients, only 70% of those who have this disease manifest typical symptoms.

Typical or esophageal symptoms concern indicators which are related with the esophagus. Such symptoms include the following:

Heartburn. This is a condition in which the patient feels a painful burning feeling in the esophagus. The pain often develops in the chest and may swell to the neck or throat. This is most probable to occur in relation with these activities: after a heavy meal, lifting, bending over, and lying down. Based on one study, about 75% of acid reflux patients experience this symptom at night. These nigh-time patients also tend to experience more harsh pain than those whose symptom occurs at other times.

Dyspepsia. Researches show that about half of acid reflux patients have dyspepsia. This is a syndrome which consists of pain and distress in the upper abdomen, nausea after a meal, and stomach fullness. It is not a rule however, that those who have dyspepsia have acid reflux.

Regurgitation. This is when the gastric contents back up into the pharynx and sometimes as far as the mouth. In cases where the acids have spilled into the tracheobronchial tree, respiratory complications can be stimulated.

There are many instances, though, that acid reflux patients do not manifest symptoms such as regurgitation and heartburn. Instead, they experience atypical or extraesophageal symptoms which include the following:

Throat Symptoms. Although it does not commonly happen, acid reflux patients suffer from symptoms that occur in the throat. Patients can also have difficulty in swallowing, a condition known as dysphagia. In critical cases, the food may get trapped in the throat or even choke, which can result to a severe chest pain. Other throat symptoms are chronic sore throat and persistent hiccups.

Vomiting and Nausea. When a patient suffers from nausea which persists for weeks, he may have acid reflux. There are few instances where vomiting can occur as often as once a day.

Respiratory Symptoms. Coughing and wheezing are counted as respiratory symptoms. These result from the overrunning of the stomach acids into the tracheobronchial tree creating bronchoconstriction.

Acid reflux disease can last for several months if not given proper medical attention. Drug treatment may only be required for a short time. But when the symptoms tend to repetitively occur, the drug treatment may have to be reapplied.

Texans Without Health Insurance and The National Health Plan For Uninsured Americans

  It is a real tragedy the number of Texans that go without Health Insurance Protection, I support the idea that every American citizen should have Individual Health Insurance, as a basic right and privilege, Yet I am not some firm on how we should fund this coverage for those who cannot afford it or , should we even subsidize this plan for non citizens. I am a taxpayer, concerned citizen and Texan. I should qualify as one who has questions and concerns about the effect of a National Health Care Plan on my future financials. I may want to retire one day, or draw on my Social Insurance. Will there be benefits for Americans after we have paid for this plan for years. I hope so!

Recently many of us have taken a tremendous hit in our savings, and unless you have a pot of gold and a rainbow you may not weather many more blows like the one we have endured. Some of the fears I have are based on the staggering numbers of uninsured residents, largely because of illegal immigration, which makes this very difficult to sort out. Texas has some 28% of its residents between 0-64 that are uninsured and 21% under the age of 17 uninsured. Many are without Medical insurance, Dental Insurance, vision care & therapy and psychological treatment. Even if they could afford it, many either don’t have access to Health Insurance Quotes or don’t have the source to point them to the proper Health Insurance Information. Affordable Health Insurance has little value to people with no means to find it. This situation is monumental financially and even more so physically to those who have no Medical Insurance. Who has the answer and who will pay for it? The government says America can pay for it, but; where will the money come from?

Will we develop a Health Care Plan with a hole in the bottom of it? There are two major underlying issues that have to be fixed in order for Americans to fully buy into and support this legislation; they are spiraling medical costs and illegal immigration Who, if anyone; is working on a plan to shore up our borders and create an update immigration policy that reflects our current picture more accurately.

The Texas Health Insurance woes pale in comparison to our complete National Health Care scenario and possible budget to finance this National Health Care solution. The coverage hypothetically will coexist alongside private insurers and compete with them in price, as well as cover all uninsurable as well as fit into the work place group insurance picture. That is an interesting, Does anyone her know why Group Health Insurance is so costly now? The true cost in a society is people who do not support the tenants of its system in every way.

Our elderly Americans have worked hard for many years to secure whatever benefits they receive and rightfully deserve. I am talking about those who are receiving benefits and those who will benefit an never have paid. You will pay for it, especially if you are successful in life financially.

Along with the benefits there should also be co-operation and giving into the system by all who benefit. I believe that no Uninsured Americans and especially Texans without insurance should be forgotten, But, Who will pay for this thing??

Don’t leave our futures in the hands of the political machine. Whatever you’re views are you should tell your congressman how you feel. No child should be uninsured nor should any of our older Citizens be without daily care and the love of other Americans.
